SPRING ATHLETIC CLEARANCE 2025

Clearances can be done on Home Campus for Spring sports.

The Deadline for Clearance submission is 1/31/2025.

1) Update your student athlete information profile for this school year.  If new this year, you will need to create you parent account account first.

2) Print the Sports Physical Form found in the medical section

for your appointment

3) Upload completed Sport Physical Form and a picture of

your Medical Insurance Card (required items) to your profile

4)  Both Parent and Athlete read through the MHS Athletic Handbook

and acknoweldge with e-signatures

Go to the MHS Online Webstore to pay

your Sport Operation Donation and any other fees for your sport.
